APPLICATION TO REQUEST CONTINUING REVIEW OF A RESEARCH PROTOCOL INVOLVING HUMAN SUBJECTS

* Submission of this application must occur 60 days prior to the expiration of the currently existing research protocol to avoid the possibility of a lapse in IRB approval. Any research study that has not received a continuing review and written approval by the end of its current approval period is automatically considered expired and all research and research-related activities must cease.

Instructions:

  1. Download the form PDF below and answer all required fields.

  3. Sign the form. You may provide a digital signature or print the form, sign it, and scan the form. Ensure that all required signatures are there before submitting.

  4. Prepare all the additional documents required by the form.

  5. Prepare all supplementary notes and documents you chose to include.

  6. Combine all the documents into a single PDF file in the specific order listed below.

  7. Click the submit button. You will receive a confirmation email (check the spam/junk folder if you do not see it after 10  minutes)
